Position Summary:
We are seeking a motivated Occupational Therapist (OT) to join our home health team. In this role, you will assess functional needs and deliver in-home occupational therapy services to adults and older adults with a variety of physical, cognitive, and environmental challenges. Your interventions will help patients achieve greater independence in their daily lives.
Key Responsibilities:
Conduct initial assessments and create customized plans of care for each patient
Deliver evidence-based occupational therapy interventions for ADL/IADL limitations, home safety, fall risk, and cognitive function
Collaborate with physicians, nurses, physical therapists, speech therapists, and care coordinators
Provide family and caregiver training on therapy goals and safety strategies
Document evaluations, progress notes, and reassessments in the EMR system (Your Tailor Made Senior Services)
Update care plans regularly based on patient progress and changing needs
Comply with agency policies, Medicare regulations, and professional standards of care
Supervise Occupational Therapy Assistants (COTAs) as needed
Participate in interdisciplinary meetings and case conferences
Promote occupational therapy education in the community, if applicable
Requirements:
Current Occupational Therapy license in the State of Texas (Required)
Graduate of an ACOTE-accredited Occupational Therapy program
At least 1 year of OT experience (Home health or geriatrics preferred)
Strong clinical judgment and independent decision-making skills
Reliable transportation, valid driver’s license, and proof of insurance
Comfortable using EMR and mobile documentation tools
