Job Description:

We are seeking a qualified and compassionate Registered Nurse (RN) Case Manager to join our team. The RN Case Manager will play a crucial role in coordinating and providing comprehensive healthcare services to our senior clients in their homes. The ideal candidate will be a skilled clinician with a strong commitment to delivering person-centered care and ensuring the best possible outcomes for our clients.

Key Responsibilities:

  1. Assessment and Care Planning: Conduct thorough assessments of clients' health and functional status. Develop individualized care plans in collaboration with the client, their family, and the healthcare team.

  2. Coordination of Care: Coordinate and manage the delivery of healthcare services to clients, ensuring that all care providers are aligned with the care plan. Regularly evaluate and adjust the care plan as needed. Some visits will be required when not covered by a staff nurse and to perform supervisory visits. 

  3. Medication Management: Monitor administration of medications, ensure staff educate clients and their families on medication management, and ensure compliance with prescribed treatments.

  4. Monitoring and Evaluation: Monitor clients' health conditions and track progress. Identify any changes or concerns and take appropriate actions. Monitor documentation by nurses for accuracy and timeliness.

  5. Nursing Education: Make sure nursing staff is up-to-date on industry standards, on managing chronic conditions, promoting health, and providing support and guidance on self-care.

  6. Communication: Maintain open and effective communication with clients, families, physicians, and other members of the healthcare team to ensure coordinated and efficient care.

  7. Advocacy: Act as an advocate for clients, ensuring their needs and preferences are respected and met. Promote clients' rights and well-being.

Qualifications:

  • Current and valid RN license in Texas
  • Minimum of 5 years of nursing experience, preferably in home healthcare or a related setting
  • Strong clinical assessment and care planning skills
  • Excellent communication and interpersonal abilities
  • Knowledge of Medicare and Medicaid regulations is a plus
  • Compassionate and dedicated to providing quality care to seniors
